Please fill out and return the appropriate forms to the concierge at least 2 weeks prior to your arrival. Please allow time to confirm your requests and be as specific as possible in filling out your forms. Click on the WINTER or SUMMER Concierge Checklist (pdf file), print and return this completed checklist with any other attachments AT LEAST TWO WEEKS PRIOR TO YOUR ARRIVAL. Print out a PDF form, fill out and then fax in to 970-476-5548 or email to vailspa@eastwestresorts.com.
